Providence Happy Valley Full Report

You should be given care that medical research has shown works best to catch health problems early, find out what's wrong, and help the most with the fewest side effects. These quality and cost scores show how this doctor's office rated at providing quality standards of care and provided care at a reasonable cost.

What do these scores mean?

What do scores mean?

We collect data from health insurers, Medicaid and Medicare fee-for-service that shows whether doctors’ offices are providing recommended care.*

Scored well below the average for doctors' offices in the state.

Scored in the middle among all doctors' offices in the state.

Scored well above the average for doctors' offices in the state.

Some doctors' offices are not included in quality and cost scores on this page.

Learn why

Learn why some doctor's offices (or medical groups) are not included in the quality and cost scores on this page:

The office doesn't have enough doctors or primary care practitioners. An office or medical group must have at least 3 primary care professionals (doctors, nurse practitioners, or physician assistants) who provide care in order to be included.

The office didn't treat enough patients for the topic on this page. Scores are shown only if an office or medical group has treated at least 30 patients who meet requirements for that topic during the time period for reporting.

If the phrase 'Results not available' appears, it means the office did not have enough patients for one of the topics on the page, but did have enough patients for other topics on the page.

If the phrase 'Results under review' appears, it means that a doctor's office or medical group asked to have their scores be reconsidered because they believe the score may be inaccurate. The phrase 'Results under review' means the score is still being evaluated.

Find out more details on data and scoring.

*Medicare fee-for-service data is included in the following:

  • Women’s Health: Mammograms (Breast Cancer Screening)
  • Diabetes Care: Blood Sugar Test

What are cost index scores?

The cost index measures the price of services and the number and type of services that a patient uses.

This score shows whether a doctor’s patients are getting care that is:

Lower than Average Cost – cost is below the state average

Average Cost – cost is equivalent to the state average

Higher than Average Cost – cost is above the state average

These services can be received at a primary care doctor’s office, or another location like a hospital or specialist office. Primary care doctors’ offices provide their patients with basic and preventive care, and often manage referrals to other services. This measure includes all the services a patient gets across settings to show how primary care doctors are managing costs associated with their patients.

Mammogram

Testing for breast cancer using mammograms

?

×

What is being measured?

A mammogram is an x-ray of the inside of the breast to check for signs of breast cancer. When breast cancer starts, it is a very small lump and there are usually no symptoms. Mammograms can find breast cancer at an early stage when the lump is still too small for a woman or her doctor to feel it.

This quality score is based on the percentage of women patients (ages 50-74) who had a mammogram during a recent two-year period.

For this test, what is meant by better, average, and below?

For each doctor's office, the percentage of patients who got a mammogram is turned into a quality score by comparing the percentage for that doctor's office to the statewide average of 67.7%.

  • 'Better' means 79.40% or more women patients got a mammogram
  • 'Average' means between 56.2% and 79.3% of women patients got a mammogram
  • 'Below' means 56.10% or fewer women patients got a mammogram

How can you use this information?

If you are between the ages of 50-74, or there is a history of breast cancer in your family, talk with your doctor about whether a mammogram is right for you.

Scored in the middle among all doctors' offices in the state. 70.8%

Pap Test

Testing for cervical cancer using a Pap smear test

?

×

What is being measured?

A Pap test checks for signs of cancer in the cervix (the cervix is the lower part of the uterus that opens into the vagina). For this test, a sample of cells from the cervix is checked under a microscope for signs of cervical cancer. Staying up-to-date on Pap tests is important because cervical cancer usually does not cause pain or symptoms until the cancer is at a late stage.

This quality score is based on the percentage of women patients (ages 21 to 64) who got one or more Pap tests for cervical cancer during a recent 3-year period.

For this test, what is meant by better, average, and below?

For each doctor's office, the percentage of patients who got a Pap test is turned into a quality score by comparing the percentage for that doctor's office to the statewide average of 61.60%.

  • 'Better' means 72.20% or more women patients got a Pap test
  • 'Average' means between 51.2% and 72.1% of women patients got a Pap test
  • 'Below' means 51.10% or fewer women patients got a Pap test

How can you use this information?

If you are between the ages of 21 to 64, or if there is a history of cervical cancer in your family, talk with your doctor about whether a Pap test is right for you

Scored in the middle among all doctors' offices in the state. 67.6%

Chlamydia Screening

Testing for chlamydia infection which can lead to infertility

?

×

What is being measured?

Chlamydia is an infection that is spread by sexual contact. Since there are usually no symptoms, Chlamydia is sometimes called a 'silent' disease. If a person is tested and Chlamydia is found, the infection can be treated and cured with antibiotics. If a woman has Chlamydia and it is not found and treated, it can lead to pelvic inflammatory disease, which can cause infertility.

A test for Chlamydia in women involves swabbing the vagina when doing a pelvic exam and can be done at the same time a woman receives a Pap test. Because Chlamydia is most common among young women, this website's quality information looks at what percentage of young women patients at each doctor's office got this test.

The quality score is based on young women patients who made at least one visit to a doctor's office during a recent 1-year period. 'Young women patients' mean sexually active women ages 16–24.

For this test, what is meant by better, average, and below?

For each doctor's office, the percentage of patients who got a Chlamydia test is turned into a quality score by comparing the percentage for that doctor's office to the statewide average of 43.3%.

  • 'Better' means 53.50% or more women patients got a Chlamydia test
  • 'Average' means between 33.3% and 53.4% of women patients got a Chlamydia test
  • Below' means 33.20% or fewer women patients got a Chlamydia test

How can you use this information?

Both in Oregon and nationwide, the low percentages for Chlamydia testing show that this type of testing is not done nearly as often as it should be. If you are a sexually active woman between the ages of 16 and 24, talk with your doctor about whether a Chlamydia test is right for you.

Children's Health Score Clinic Score Statewide Average

Developmental Screenings

Children 1, 2 or 3 years old should have a developmental screening

?

×

What is being measured?

Providers use developmental screening to check if children are learning basic skills when they should, or if they might have problems. Your child's doctor may ask you questions or talk and play with your child during an exam to see how he or she learns, speaks, behaves, and moves. Since there is no lab or blood test to tell if your child might have a delay, the developmental screening will help identify if your child needs to see a specialist.

This quality score shows whether children who turned ages 1, 2 or 3 during the measurement year had a developmental screening in the year before their birthday.

For this test, what is meant by better, average, and below?

For each doctor's office, the percentage of children who had a developmental screening in the year before their first, second or third birthday is turned into a quality score by comparing the percentage for that doctor's office to the statewide average of 68.70%.

  • 'Better' means 88.90% or more of children had a developmental screening before their first, second or third birthday
  • 'Average' means between 48.6% and 88.8% of children had a developmental screening before their first, second or third birthday
  • 'Below' means 48.50% or fewer of children had a developmental screening before their first, second or third birthday
Scored in the middle among all doctors' offices in the state. 70.4%

Well Child: 0-15 Months

Children should have at least six visits from birth to 15 months old

?

×

What is being measured?

Routine well child visits are important for the health of your child. These visits are not the same as visits to a doctor when your child is sick. At well child visits, doctors check your child's height, weight, hearing, and vision to see how your child is developing. Your child's doctor will also ask questions about how your child is eating and behaving, and provide important preventive care, such as immunizations (shots to protect against childhood diseases). Well child visits are an opportunity for you to discuss questions and concerns that you may not be able to talk about during your child's sick visits.

This quality score shows whether children up to 15 months old had at least six of the eight recommended well child visits at the doctors' office.

For this test, what is meant by better, average, and below?

For each doctor's office, the percentage of well child visits for children 0-15 months old is turned into a quality score by comparing the percentage for that doctor's office to the statewide average of 70.80% .

  • 'Better' means 82.70% or more of children had six or more well child visits
  • 'Average' means between 59.0% and 82.6% of children had six or more well child visits
  • 'Below' means 58.90% or fewer of children had six or more well child visits

Adolescent Well Care

Adolescents 12 to 21 should have one visit per year

?

×

Blood Sugar Test

Measures level of blood sugar during past 3 months

?

×

What is being measured?

The 'A-1-C' test is a blood test that measures the average level of blood sugar during the past 2 or 3 months. If you have diabetes, the A-1-C test is different from blood testing you might do yourself. When you test your blood, you find out what your blood sugar is at the moment you do the test. But when you have the A-1-C laboratory blood test, you find out how well your blood sugar has been controlled, on average, over a period of time.

Often, there are no symptoms to warn people when their blood sugar has been staying too high. To know for sure whether blood sugar is staying at a healthy level or not, people with diabetes need to have an A-1-C test at least twice a year. Keeping blood sugar close to normal helps prevent or delay the health problems that can happen when people have diabetes.

The quality score is based on the percentage of adult patients (ages 18-75) with diabetes who got the A-1-C blood sugar test at least once during a recent one-year period.

For this test, what is meant by better, average, and below?

For each doctor's office, the percentage of patients with diabetes who got an A-1-C test is turned into a quality score by comparing the percentage for that doctor's office to the statewide average of 87.00%.

  • 'Better' means 94.10% of patients got the A-1-C test
  • 'Average' means between 80.0% and 94.0% of patients got the A-1-C test
  • 'Below means 79.90% or fewer of patients got the A-1-C test

How can you use this information?

If you have diabetes, talk with your doctor about whether it is time for your next A-1-C blood sugar test.
